In 2014 the Journal of Prostate Cancer and Prostatic Disease, researchers Cui et al., conducted a meta-analysis which showed no difference in prostate cancer rates rates for men who received testosterone vs placebo.

In 2016 Lancet Diab Endocrinology, researchers Wallis et al., demonstrated a greater reduction in prostate cancer risk with greater duration testosterone therapy.

In 2023 Harvard Medical School Symposium, Dr. Abraham Morgentaler stated we do not have evidence to support high endogenous testosterone or testosterone therapy increases prostate cancer.

There are no randomized controlled large studies demonstrating safety yet.

Cardiovascular Considerations

In 2009 Journal of Impotence Research, researchers Maggio et al., reported low serum testosterone levels are an independent risk factor of cardiovascular and all-cause mortality.

In 2011 Journal of American College Cardiology, researchers Ohlsson et al., found higher serum testosterone levels are associated with a reduced risk of fatal and nonfatal CV events in community dwelling elderly men.

In 2015 Mayo Clinical Proceedings, Morgentaler et al. reports, “In summary, there is no convincing evidence of increased CV risks with T therapy. On the contrary, there appears to be a strong beneficial relationship between normal testosterone and cardiovascular health that has not yet been widely appreciated.”

In 2022 Elsevier, Hudson et al. meta-analysis of more than 3000 patients, “We found no evidence that testosterone increased short-term to medium-term cardiovascular risks in men with hypogonadism.”

In conclusion, we know there is a strong correlation for low testosterone and increased cardiovascular events, and conversely, a growing body of evidence showing that healthy testosterone levels have cardioprotective benefits.

The safety of women receiving hormone therapy has also been widely and seemingly unfairly scrutinized.

In 2002 a media frenzy ensued when the Women’s Health Initiative reported an increased risk for cancer and cardiovascular events for female’s receiving hormone replacement therapy, obscuring the fact that the reported excess risk was clinically meaningless, at two events per 1,000 person-years. The therapy in question was a conjugated estrogen pill made from mare urine. Follow-up studies failed to support these finding. Despite that, the damage was done, and all forms of female hormonal therapies were drastically halted.

In 2021 the results of the largest long-term study, The Testosterone Therapy and Breast Cancer Incidence Study, demonstrated that testosterone or combination testosterone and estrogen therapy significantly reduced the incidence of breast cancer in perimenopausal and postmenopausal women. This is the second multi-year long-term study demonstrating that testosterone demonstrates breast protective benefits.

Since the 1930’s it was known that testosterone therapy was beneficial for pre and postmenopausal women. By 1947 there was enough evidence to suggest it was an effective and appropriate treatment for women, yet in the proceeding decades, medical and societal bias persisted.

We agree fully with the following passage by Abraham Morgentaler, “The true outrage is that social forces and hysteria have combined to deprive men (and women) of a useful treatment without regard for medical science.” Morgentaler A. Testosterone, Cardiovascular Risk, and Hormonophobia. J Sex Med 2014;11:1362–1366.
